In the orchiectomy patient group, 11/41 patients were hypergonadotropic, and 9/11 patients hypergonadotropic hypogonadal orchiectomy patients were biochemical non‐responders. Some treatments for low testosterone can reduce fertility, while others may help preserve it. Sometimes, other medications are added to control estrogen or support sperm production. This combination can help reduce some of the side effects linked to higher TRT doses, such as shrinking testicles or reduced sperm production. The dosage depends on the person’s age, weight, blood levels, and symptoms. Clomid (clomiphene citrate) is usually prescribed off-label for men with low testosterone who want to maintain fertility. Other men may start with testosterone therapy, but find that their sperm count drops. Clomid helps keep estrogen levels in check because it blocks estrogen receptors in the brain. Too much estrogen in men can cause side effects such as breast tenderness, water retention, or mood swings. Testosterone can change into estrogen in the body through a process called aromatization. It supports the brain’s natural signals to the testicles, which helps keep sperm production going. In men, too much estrogen can tell the brain to slow down testosterone production. When levels drop, the brain increases production again. Monitoring helps to adjust the dose and avoid risks such as high red blood cell count, increased estrogen, or sleep apnea worsening. Each method delivers testosterone in a slightly different way, but all aim to raise blood levels back into the normal range.
