There's more to testosterone than guys behaving badly. For example, did you know that testosterone is a key player in prostate cancer? When you think of testosterone, what comes to mind? The mechanism of activating the promoter for SHBG in the liver involves hepatocyte nuclear factor 4 alpha (HNF4A) binding to a DR1-like cis-element which then stimulates production. Health effects include a lowered risk of endometrial cancer and an increased risk of systemic lupus erythematosus. SHBG is produced mostly by the liver and is released into the bloodstream. DHT binds to SHBG with about 5 times the affinity of testosterone and about 20 times the affinity of estradiol. SHBG's production is regulated under the influence of FSH on Sertoli cells, enhanced by insulin, retinol, and testosterone. Testosterone and estradiol circulate in the bloodstream, loosely bound mostly to serum albumin (~54%), and to a lesser extent bound tightly to SHBG (~44%). When produced by the Sertoli cells in the seminiferous tubules of the testis, it is called androgen-binding protein (ABP). A brief blood‑pressure check, review of medication interactions (e.g., rifampin), and assessment of liver function are typically sufficient; routine pelvic exams or laboratory panels are not required in healthy women. Combined oral contraceptives (COCs) are often chosen for acne, but they also provide several non‑acne health advantages that can be part of a holistic skin‑care plan. Accutane is generally reserved for cases that have not responded to topical agents, antibiotics, or hormonal therapy, or when acne is predominantly cystic and scarring is a concern. For mild‑to‑moderate acne or for patients who need a contraceptive, hormonal therapy is usually tried first because it is safer and easier to manage. The FDA‑approved acne‑treating pills—Ortho Tri‑Cyclen (norgestimate), Yaz (drospirenone), Estrostep FE, and Beyaz—work by lowering androgen levels, which reduces sebum production and helps clear deep, inflammatory lesions. Norgestimate has weak intrinsic androgenic activity but still raises sex‑hormone‑binding globulin (SHBG) and modestly blocks androgen receptors. Lifestyle counseling for holistic skin health – Successful skin outcomes depend on more than medication. Progestin‑only methods (mini‑pills, injections, IUDs) can actually worsen acne and are not recommended for this purpose. It works by shrinking sebaceous glands, normalizing follicular keratinization, and exerting strong anti‑inflammatory effects. For patients with severe cystic acne or scarring, systemic isotretinoin (Accutane) remains the most potent option. In practice, once a woman has tried topical agents and oral antibiotics without satisfactory improvement, a COC is often the next step because it also provides reliable contraception. Best contraceptive pill for skin – Combination estrogen‑progestin pills are the most effective for skin health. Progestin‑only "mini‑pills" lack estrogen, may increase free androgens, and can worsen acne. When acne‑prone women need reliable contraception, combined oral contraceptives (COCs) are the only FDA‑approved birth‑control option that also improves skin. Having most of your testosterone bound to proteins makes sure your body doesn't use too much. For example, higher estrogen levels, commonly observed in women, especially during pregnancy, can increase SHBG production. Factors such as age, gender, body mass index (BMI), diet, and certain medical conditions can influence SHBG levels. This is particularly important in scenarios where the balance of hormones is critical, such as during puberty, pregnancy, or in the management of hormone-related disorders. By binding to sex hormones, it effectively regulates their availability to tissues and organs. The binding of hormones to SHBG is a dynamic process, and the equilibrium between bound and unbound (free) hormones is crucial for maintaining hormonal balance. It functions like a bus, carrying these hormones through the bloodstream to various parts of the body. Structurally, SHBG is a relatively small protein with a specific binding site for sex hormones. The mechanism for the decline in SHBG during puberty is unclear; it was suggested that decreases in SHBG are partly due to increases in testosterone (51). Considering that it is probably an indirect effect of testosterone on SHBG production, and could be affected by other mediators, the influence of testosterone on SHBG appears to be minimal during early puberty. This appears contrary to the classical postulation that testosterone inhibits SHBG production, which was mainly supported by studies of androgen treatments in males and in human liver cells (6, 14, 15, 49). In addition to the influence of SHBG on total-T, our results of mediation models and Mendelian randomization also indicated a possible—albeit somewhat weaker—positive effect of total-T on SHBG in late puberty and adulthood. Changes in pro- or anti-inflammatory cytokines and alteration in the transcription factors (such as HNF-4α and PPAR-γ) of SHBG gene that occur in obesity could account for the reduction in plasma levels of SHBG (47, 48). Men who watch sexually explicit films also report increased motivation and competitiveness, and decreased exhaustion. Therefore, these mammals may provide a model for studying clinical populations among humans with sexual arousal deficits such as hypoactive sexual desire disorder. Testosterone treatment for reasons other than possible improvement of sexual dysfunction may not be recommended. In women, correlations may exist between positive orgasm experience and testosterone levels. 2020 guidelines from the American College of Physicians support the discussion of testosterone treatment in adult men with age-related low levels of testosterone who have sexual dysfunction. This is known as hormone replacement therapy (HRT) or testosterone replacement therapy (TRT), which maintains serum testosterone levels in the normal range.
